I have read and agree to the following:

I am enrolling in an online fitness workout of strenuous physical activity which may include but is not limited to aerobic conditioning and cardiovascular conditioning, weight training, strength training, and flexibility training offered by Mind N Body Fitness and its owner ZeZor DeForce. I hereby affirm that I am in good physical condition and do not suffer from any disability that would prevent or limit my participation in the online fitness workout. I acknowledge that I should consult with my physician before beginning any type of fitness workout as factors unknown to me may have an adverse affect on my physical well-being, including death. I have either had a physical examination and have been given my Physician’s permission to participate in a fitness workout, or I have decided to participate in a fitness workout without the permission, approval, or consent of my Physician and do hereby assume all responsibility for my participation.

In consideration of my participation in this online fitness workout, I, for myself, my heirs and assigns, hereby release Mind N Body Fitness and its owner ZeZor DeForce, or any persons involved with the online fitness workout from any claims, demands and causes of action arising from my participation in the online fitness workout. I fully understand that I may injure myself as a result of my participation in the online fitness workout and I do hereby release Mind N Body Fitness and its owner ZeZor DeForce, or any persons involved with the online fitness workout from any liability now or in the future including but not limited to heart attacks, muscle strains, pulls or tears, broken bones, shin splints, heat prostration, knee/back/foot injuries and any other illness, soreness or injury however caused, occurring during or after my participation in the online fitness workout... By checking the box above, I hereby affirm that I have read and understand the above.
